1. Participant Details

I confirm that I am the parent or legal guardian of the child named in this form.

 

2. Acknowledgement of Risk

I understand that participation in martial arts and physical activity involves inherent risks, including but not limited to minor injuries such as bumps, bruises, and strains.

 

3. Medical Declaration

I confirm that my child is in good health and able to participate in physical activity.
I will inform Roldan Muay Thai of any medical conditions, injuries, or concerns prior to participation.

 

4. Release of Liability

To the extent permitted by Australian law, I release and hold harmless Roldan Muay Thai, its coaches, and staff from any liability arising from injury, loss, or damage sustained during participation.

 

5. Behaviour & Safety

I understand that all participants must follow coach instructions and safety rules at all times.
Failure to do so may result in removal from the session.

 

6. Emergency Consent

In the event of an emergency, I authorise staff to seek medical assistance for my child if required.

 

7. Media Consent

I consent to photos/videos of my child being used for promotional purposes

 

8. Agreement
